Set of documents required for the establishment of cooperation concerning the provision of services shall comprise the following:
- contract for the provision of telecommunications services
- order, depending on the type of the service
- 2 x pre-selection order
- xerox copy of the Tax Identification Number NIP certificate
- xerox copy of the Company Statistical Number REGON certificate
- xerox copy of the National Court Register entry (valid 3 months following the date of issue) or certificate of entry into the Commercial Register
- invoices issued by TP S.A.
